I would like know How to add custom adobe form for Dunning. And how the data will be mapped the Adobe form from dunning procedure.

Could you please provide me the doc. or step by step process. I have created Adobe form, but i don't know how to map the data to the form from dunning procedure. What parameters should be included in Adobe forms to get the data from Dunning procedure.

Please give me the suggestions.
